To my knowledge, you cannot control how voiceover speaks notifications when the phone is unlocked. However, there is a setting to adjust what happens when a notification comes in on the lock screen. Go into voiceover settings, and then find the item calledspeak notifications on lock screen.

It sounds like you are accidentally touching the screen instead of flicking. In iOS7, the message body takes up just about the entire screen, so that touching anywhere will cause your cursor to move. Try doing all gestures on the keyboard, so that the worst you’ll get is an extra character

Yes, you can use the rotor. Go into VO'S settings, choose rotor, find sounds, and double tap it to add it. Now, anywhere in iOS, just rotor to sounds and flick up or down to turn them on or off. I wish there were a better way, but for now that is the best we can do. Sent from my iPhone On

That's because the auto refresh feature is for refreshing app content in the background. When enabled, apps that support it can pull data every so often, so there's less of a wait when you launch the app since most or all of the data you'd normally have to wait for is already waiting for you.
